Case Management

Case managers assist individuals on a waiver with coordinating services and developing a person-centered and goal-directed plan of care. They also assist with accessing, coordinating, and monitoring needed services as they relate to the individual's assessed needs and preferences. The case manager provides regular visits to review the plan of care and make changes to services as needed.
